As part of your health benefits plan, you now have a Shared Health Clinical Health Record® (CHR), which will keep track of treatment histories, medications, immunizations, and allergies for you and your family. From now on, mention that you have a Shared Health CHR™ to any doctor or specialist you see. They'll be able to access your health information at the point of care when it is needed and make sure the information is updated. That way you get the best care possible.
